EXPLICIT CONSENT STATEMENT

This General Information Text on the Protection of Personal Data and the Company's Regulation on the Protection and Processing of Personal Data are provided to you by Tansan Private Health Services and Trade Ltd. Co. (the COMPANY) in accordance with the relevant provisions of the Law No. 6698 on the Protection of Personal Data (KVKK) and the Company's Regulation on the Protection and Processing of Personal Data.,

Personal data is processed by the Company, acting as the data controller, or by data processors authorized by the Company after taking the necessary security measures, for the purpose of providing the Company's healthcare services to its patients under the best conditions, ensuring quality, reliability, effectiveness and uninterrupted access to healthcare services, providing patient support, responding to emergencies, communication, security measures, fulfilling legal obligations, ensuring data security, maximizing patient satisfaction, and thus fulfilling the requirements of all healthcare services in general. This includes personal data directly or indirectly related to identity information, address information, contact information, legal correspondence and case file information, visual and audio recordings, patient files and other personal data, information on disability status, blood type information, information on devices and prostheses used, allergies, diseases, diagnoses, examinations, treatments applied, medications used, tests, films and graphs, and all other special categories of personal data. This data is retained primarily for the period stipulated in the legislation or necessary for the purpose for which it is processed, in accordance with the principles of the Personal Data Protection Law. I acknowledge and declare that I have been informed and enlightened by the Company that my personal data and special categories of personal data may be processed, obtained, recorded, securely stored in physical or electronic form for an appropriate period of time for the purpose of processing, maintained, modified, reorganized, disclosed in accordance with the legislation, transferred, acquired, classified, processed, or the use of data may be prevented in accordance with the general principles stated in Article 4; furthermore, that all my personal data and special categories of personal data will be recorded, processed, stored, and transferred for the duration of the healthcare service and, in any case, for the legal retention period determined within the framework of the relevant legislation, and that after the expiration of the legal retention period, all my personal data will be deleted, destroyed, or anonymized in accordance with the Personal Data Protection Law and its subordinate legal regulations, and that I have given my explicit consent within the framework of the Personal Data Protection Law.

I acknowledge and explicitly consent to the transfer of my personal data by the Company to its employees, officers, shareholders, legally authorized public institutions and organizations, natural persons or private legal entities, and business partners and service providers with whom the Company collaborates or receives services for the performance of the services provided to me, provided that this transfer is limited to the purposes stated above.

However, in accordance with Article 11 of the Personal Data Protection Law and related legislation, I have the right to apply to the Company to learn whether my personal data is being processed, to request information regarding the processing of my personal data if it has been processed, to learn the purpose of the processing of my personal data and whether it is being used in accordance with its purpose, to know the third parties to whom my personal data has been transferred domestically or abroad, to request the correction of my personal data if it has been processed incompletely or incorrectly, to request the deletion or destruction of my personal data if the reasons requiring the processing of this data cease to exist, to request that these correction and deletion requests be notified to the third parties to whom the personal data has been transferred, to object to a result that is detrimental to me arising from the analysis of the processed data exclusively through automated systems, and to request compensation for damages if I have suffered damage due to the unlawful processing of my personal data. To exercise these rights, I must submit my request, signed and including the necessary information to identify my identity and explanations regarding the right I wish to exercise, in writing to Teşvikiye Mah. Hakkı Yeten Cad. I acknowledge that I have the right to personally deliver my identity verification documents to the Company Directorate located at Doğu İş Merkezi No: 15/8 Şişli-İstanbul, or via a notary public, or through other methods specified in the Personal Data Protection Law.

Furthermore, I acknowledge and declare that the personal data I have shared with the Company is accurate and up-to-date; and that I will notify the Company of any changes to this information.

I hereby give my explicit consent to the processing, use, and sharing of my personal data, including my special categories of personal data as defined in the Personal Data Protection Law, within the scope of the relevant process and limited to the purpose of processing, and to its storage for the necessary period; I also acknowledge that I have been adequately informed about this matter; and that I have read and understood this text, the Company's Personal Data Protection and Processing Regulation, and the Disclosure Statement.;
